
1. To validate, complete this request form and mail within 60 days from date of issue, or this offer will be void. Holiday Travel of America will send an acknowledgment letter, requesting your two travel dates, a $50.00 per person processing fee, and estimated port charges and government taxes of $146.21 per person for Port Canaveral, FL or $142.60 per person for Los Angeles, CA (subject to change). To complete the validation process, you must submit your tax deposit within 21 days of the date of that acknowledgement letter.
2. When you receive your letter, select two valid dates at least 21 days between choices in chronological order. These dates must be for departure on Monday’s from Los Angeles*, CA, and Sunday’s from Port Canaveral, Fl. Holiday Travel of America must receive these dates at least 60 days in advance of the earliest departure date with all applicable port charges and government taxes. *Departs Long Beach Terminal.
3. Passenger(s) traveling on this offer must be listed on the original reservation request form, and must be at least 21 years of age. This offer is non-transferable, and identification will be required.
4. Your certificate is valid for passage for two adults to cruise. Airline tickets, airport transfers, taxes, surcharges, security fees and miscellaneous expenses are not included. Alcoholic beverages are not included in this offer.
5. Staterooms are Category 1 Upper/Lower (bunk beds). Passengers may be traveling with a group, and at times be required to stay together as a group. However, reservations are accepted on an individual basis only. Two or more request forms received together, and requesting the same travel dates will not be accepted.
6. This offer is valid for travel during the following periods in 2009: Jan 6-Jan 17, Jan 25- Feb 10, Feb 22-April 11, April 19-May 23, May 31- June 6, Sept 8-Nov 21, Dec 1-13, subject to our ability to fulfill the reservation request at its contracted rate for the time and destination chosen. Dependent upon date of issue, this offer is valid for travel during the following periods in 2010: Jan 5-Jan 17, Jan 25-Feb 11, Feb 23-Mar 27, April 11-May 24, Sept 8-Nov 20, Nov 29-Dec 13. This offer is also subject to high season blocked out periods.
7. Cancellation & Change Policy: Any confirmed reservation that is changed will incur a fee. The following is the policy: Changes to confirmed reservations, or cancellations, from Holiday Travel of America requires a $50.00 fee. Cancellations of confirmed reservations from the cruise line up to 61 days from departure date is complimentary, 60 to 30 days from departure date is the amount of the deposit (Minimum $300.00), 29 to 8 days prior to departure is an additional $250.00, or the balance paid, whichever is less, and 7 days or less from departure, no refund. All additional cancellation/change fees incurred will need to be paid prior to change of a confirmed reservation. Cancelled confirmations will result in voided certificates.
8. This cruise offer cannot be sold, bartered, exchanged, or transferred to another party.
9. Third and fourth person upgrade, when available, must be purchased through Holiday Travel of America at cruise lines annual list rates. Weekend upgrades are available.

11. This offer is not valid for groups, and cannot be used in conjunction with any other cruise offer. This program is non-transferable, cannot be sold to the end user, and cannot be redeemed for cash. It is important that you do not leave for your cruise until you have received your confirmation voucher. Airfare is available, and can be purchased through Holiday Travel of America, or an assigned travel agent.
12. This offer is void where prohibited by law. Holiday Travel of America is not responsible for any verbal or written representation, by any person or company, which is not contained in this certificate. Package subject to change without notice. For promotional use only. Not for sale. Due to the promotional nature of this offer, Holiday Travel of America can only honor one certificate, per household, per year.
13. Holiday Travel of America is not to be held responsible for any act of God, and/or any other circumstances beyond our control, causing cruises to become unavailable.
14. Both passengers must sign the form on the front side or this certificate is void.
15. This certificate expires one year from date of issue.
