$25 Gift Card Terms & Conditions

Please read the offer details completely.  Failure to follow the instructions properly will result in your offer becoming void and resubmissions will not be accepted.

1. Please complete the Gift Card Claim Voucher attached and send it in an envelope by certified mail only, which must be postmarked within 21 days from the sponsors issue date.  This offer is NONTRANSFERRABLE and you must provide a photocopy of a government issued photo identification to confirm the promotional offer was not transferred to a third party.  Upon validation of the form, HTOA, Inc. will then mail you a Gift Card Request Form, including the terms and conditions of this offer and a request for a nominal shipping and handling of $5.95.

2. The gift cards included in this offer are: Target®, Wal-Mart®, The Home Depot®, Best Buy®, Red Lobster®, Olive Garden®, Outback Steakhouse® or A Suite Week®.  A Suite Week® offers 7 nights of discounted accommodations in a hotel or condo.

3. This offer is void where prohibited by law and not available to California residents.  This offer may not be sold retail to the end-user.

4. Recipient must sign the form on the reverse side acknowledging that they have read, agree and understand the Offer Details or this offer is void.

5. HTOA is not responsible for any verbal or written representation, by any person or company, which is not contained in this certificate.

6. Please allow 6 - 8 weeks for delivery of your gift card of choice, which will be sent to the address on your Gift Card Claim Voucher via certified mail.

7. One certificate per household, per family, per year may be redeemed.
